No, you could do it with just flash and some sort of middleware solution. I
will see if I can throw together something that will show this kind of thing
off.

That would (as far as I know) require the use of Macromedia's Multiuser
Server - a 50-user license comes with Director 7 but allows over the net
multiplay/chat. Check out the i-Sketch link on the sites listed at that URL
below.
