FLASH: ezformml.cgi

Hi, I've a problem to put the above subject working, please advise.

Attached is the copy of my cgi form, will you kindly check it for me

what exactly gone wrong for I've tried in a lot of different

possibilities and I've double check the path of the file....... well I

really don't know why it doesn't work. I know I'm not a programmer, so

I've follow the instruction very carefully and I've read it like a

thousand times that I haven't missed anything. Anyway, for your

reference I'll send you the instruction as well.

I will be very appreciate if you could solve this problem for me.
