I can think of a few ways to get similar results:

-- can happen with a damaged ActiveX installation (assuming you're on
Windows!)... people often tell each other to remove parts of the
installation but that can break things, better to use the uninstaller
instead

-- can happen if a certain site's JavaScript detection is written to not
detect new versions... popular detection routines are used more often than
handrolled ones (the one-or-all-sites test could help reveal this

-- could also be something else at play... we online may be picturing the
wrong symptom from the description (for instance, in IE/Win you wouldn't
see any "plugin download dialog", because IE/Win doesn't use plugins for
well-tagged sites)
