The ActiveX control is actually installed into the Windows NT registry. You need to do more than just delete the file to make it really be uninstalled. Within Netscape, you can just remove the NPSWF32.DLL from the Netscape plugins folder; within Internet Explorer you need to do a bit more.
Check the tech note for uninstalling the Flash ActiveX control:

By the way, if you get an error like "(loadLibrary("swflash.ocx") failed. GetLastError returns 0x0000007e)" it means you didn't specify the path to the swflash.ocx file properly.
