I'd suggest a text entry box on the homepage for the e-mail address. On
submitting the form the user a new page is delivered that:

a) Informs the user that they have been added to the list.
b) Provides a summary of the site's privacy policy and how to unsubscribe.
c) Displays an electronic form that allows the user to submit further
information such as age, sex, etc, should they wish to.
